New patient paperwork

Get a head start on your paperwork before you come in! You have two options, fill the forms out to the best of your knowledge – if you are unsure about something, just leave it blank.

Cancellation policy

We ask for 48 hours notice prior to cancellation or rescheduling.

Our cancellation policy is in place out of respect for all of us, including you. Cancellations with less than 48 hours notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule into that time slot. For this reason we ask for 48 hours prior to cancellation or rescheduling. If not, a $75 fee will be automatically charged per hour of treatment.
